Hazeldenes have registered for the Australian Governments Job Keeper Program, the Australian Taxation Office (ATO) requires eligible employees to document whether or not they wish to be nominated to receive Job Keeper payments. Please complete the nomination form and forward to email@example.com
The Nomination Process:
Step 1 – Download the pdf nomination form: Download Now
Step 2 – Open the form in a pdf reader such as Adobe Acrobat.

Step 3 – Please read through each question carefully and complete only the questions that are relevant to you.
Step 4 – You are not required to sign the form just make sure that you have completed all relevant sections and dated it as per the format prescribed.
Step 5 – Save and Email. See diagram below. If you are having difficulties completing the form please contact the HR Department at Hazeldenes. All forms need to be emailed to: firstname.lastname@example.org Please include your full name and the department area that you are employed in your email.
